How do I make an appointment? |

Call or email me at the Balance Center. You will need to fill out a New Patient Intake Kit prior to your first visit. This packet takes several days to fill out as you must record your diet . Once completed, this packet must be mailed to me (no signature required) along with a small hair sample and a labeled, sample of anything you are currently taking. Recent medical test results are also appreciated. This packet must arrive 2 days prior to your telephone appointment, or be brought with you at the time of your in office appointment.
